Skilled Care
Our skilled nursing service’s resident-focused approach provides those who have suffered from a prolonged acute or chronic illness or have recently been injured or undergone surgery with the best care possible. In these cases, residents' conditions are often stable, but they continue to require assessment and treatment. And whether it’s nutritional support or assistance with self-care and the activities of daily living, our team is committed to meeting the needs of each resident at every stage of recovery.
Our full range of rehabilitative services assist residents with physical, cognitive, psychosocial, and/or communication impairments. Respiratory, physical, occupational, and speech therapy programs use the coordinated efforts of an interdisciplinary team to contribute to each resident's rehabilitation – helping to ensure a successful return to home or work.
We also offer specialized services at an intensity level based on each resident's unique needs. These benefit residents of all ages who are admitted directly from a hospital ICU setting, physician's office, home health, emergency room, or medical/surgical settings.
Primary and specialist physicians, nurse practitioners, and physician assistants lead our interdisciplinary clinical teams, following residents throughout their recovery. These teams enable us to provide a full range of care management services that include case management and discharge planning.
Long Term Care
If your situation requires a level of care beyond that which can be provided at home or via an assisted living facility (due to illness, injury, frailty, or other limitations), our long-term care facilities can be the solution. Our nursing facilities employ licensed and certified nurses, therapists, and certified nursing assistants. Your attending physician, in collaboration with an interdisciplinary care team, can develop a specialized care program that will address your individual needs.
We’re accustomed to the unique challenges associated with long-term care – we strive to provide care to our residents in the best way possible. All nursing facilities are licensed and operate under the direction of a licensed nursing home administrator.
Assisted Living
The philosophy behind our assisted living program is simple – to establish an environment for mature adults where they can enjoy life to its fullest, while still receiving the assistance they might need. Here, they can remain as independent as possible while enjoying their privacy and peace of mind.
Our assisted living facilities place special emphasis on providing an atmosphere of respect and friendship where residents can get the help they want, when they need it. Supportive services can include assistance with daily personal care, reminders to take medications, or housekeeping. Consulate’s friendly, helpful staff is available 24 hours a day.
Independent Living Care
Retirement often brings with it the need to simplify. You want to make time for what’s really important in life. Consulate’s independent living units can help seniors achieve that goal by providing a blend of security, exciting activities, comfortable, affordable accommodations, and special amenities.
Our independent living units offer spacious apartments and/or individual homes designed to ensure maximum privacy and comfort. We also offer extra features including beautifully landscaped grounds, housekeeping services, and scheduled transportation.
Consulate’s retirement units provide a friendly mix of individuals with diverse lifestyles and experiences – the majority of our residents stay active within the retirement and local communities.
Alzheimer's Care
Some facilities offer specialized services dedicated to meeting the unique needs of those with Alzheimer’s disease. Supportive care and success-oriented activities can help maintain self-esteem while working on physical, cognitive, and psychosocial abilities. And structured group activities can help provide a much-needed routine, while small group and one-on-one sessions work to meet individual needs.

Individuality is cherished and nourished. Every individual’s abilities and potential skills are encouraged and celebrated. We are dedicated to preserving, moment by moment, an individual’s identity, self-esteem, and dignity. Dementia-specific care is provided by a knowledgeable, skilled, and compassionate staff. We have a program to help residents that wander. We are proud of the program and it has produced positive results, but we cannot guarantee it stops every resident from wandering every time.
