Consulate Health Care centers offer post-acute services for those patients who no longer require an in-patient hospital stay but who are not ready to go home. Our outcome-oriented rehabilitation therapy programs help patients rebuild the strength, function and mobility needed for a successful return home. Comprehensive rehabilitation services are provided in-house at every Consulate Health Care center.

Rehabilitation services include Physical Therapy, Occupational Therapy and Speech Therapy. Our rehabilitation professionals are licensed therapists who are specially trained to meet the complex needs of patients recovering from a wide range of illnesses or injuries.

Alzheimer's disease and other types of dementia can have a devastating impact - not only for the patient but also for their family members.

For family members, caring for someone with Alzheimer's disease at home can become overwhelming and often become no longer possible as the disease progresses. We understand both the emotional and physical challenges that come with Alzheimer’s disease and other dementias, and we’re here to help.

Many of our senior healthcare centers offer specialized care for patients with Alzheimer’s disease and other forms of dementia and memory loss.

The philosophy behind our assisted living program is simple – to establish an environment for mature adults where they can enjoy life to its fullest, while still receiving the assistance they might need. Here, they can remain as independent as possible while enjoying their privacy and peace of mind.

Our assisted living centers place special emphasis on providing an atmosphere of respect and friendship where residents can get the help they want, when they need it. Supportive services can include assistance with daily personal care, reminders to take medications, or housekeeping. Consulate’s friendly, healthcare team is available 24-hours a day.
